Q. What is the Menopause?

A. Strictly speaking, the Menopause means cessation of periods. However, for a variable period of a few years before, Ovarian function declines increasing production of FSH (Follicle Stimulating Hormone) which stimulates Oestrogen output. This period of hormonal imbalance prior to cessation of periods is generally known as the Peri Menopause.  

Q. How is the Menopause diagnosed?

A. Usually on the history of irregular or missed periods, hot flushes etc.  However, it can be confirmed by measuring the FSH level.

Q. What symptoms am I likely to get?

A. Classical Menopause symptoms include: hot flushes, sweats, vaginal dryness, poor energy, joint pains, weight gain and headaches.  Approx. 30-40% of women may have no symptoms at all.  Hot flushes should disappear in 70-80% of women two years after the onset of the menopause.

Q. Should I take HRT?

A. This is one of the biggest health decisions facing women, and choice is individual.  

Because of increase in Breast Cancer risk and Heart Disease, HRT use is now usually restricted to short term (1-2 years) relief of menopausal symptoms.

Q. What options are there to HRT?

A. Complementary Medicine can help with many of the symptoms around the Menopause.  Treatments are individually selected, but include:

1.    Herbal treatment.  These have a long history of usage around the Menopause and can be used either singly or combined depending on history and symptoms.  Herbal preparations include Black Cohosh, Vitex Agnus Castus, Phyto Soy, Dong Quai and Milk Thistle.  Theses have different functions, but generally aim to stabilise hormonal levels and support the liver, an important organ of hormone excretion.  It is very important with these preparations to use good quality brands at the right dosage for an appropriate period of time.

2.    Vitamin/Mineral supplements.  These are individually selected, but can include Essential fatty acids, Magnesium, B Vitamins, Zinc, Boron, Bioflavonoids etc.  I would again stress the need for correct dosage and be aware of interactions with other drugs and medical conditions.

3.    Correction of Dietary imbalances/food intolerances.  Advice is given in relation to dietary change including avoidance of foods precipitating hot flushes.  Allergy/food intolerances, Asthma, Eczema, Cystitis and Migraine all have a tendency to increase at the Menopause.  Detection and correction of these can significantly improve overall symptom control and general health and well being.

4.    Acupuncture/Homeopathy.  Can be of benefit in many conditions around Menopause e.g. muscle/joint pain, bowel complaints, Migraine, hot flushes and low energy.

5.    Yeast/Sugar Sensitivity (Candida Syndrome).   With the advent of antibiotics, the contraceptive pill and increased sugar intake, this has become increasingly common.  Symptoms of this condition include a tendency to recurrent thrush/fungal infections, increased sugar cravings, poor alcohol tolerance, irritable bowel syndrome, increased pre menstrual tension and generally reduced energy and well being.  If this condition is present at the Menopause, correction of it can be very beneficial both for hormonal related symptoms and overall health and well being.  Treatment programme includes:

a.     Diet – reduction of sugar and yeast products.

b.    Use of Vitamin and Mineral Supplements.

c.     Anti-fungal Medication.

d.    Herbal Medicine/Homeopathy/Acupuncture

Q. Are other conditions more common at the Menopause?

A. Yes.  Cystitis, Thrush and Allergy related problems e.g. Asthma, Eczema and Migraine can increase at times of hormonal change.  These generally respond well to treatment.
